*DIAMOND MEMBERSHIP TERMS & CONDITIONS:
Memberships are non-transferable and non-refundable. Benefits only applicable to member (no gifts). Terms and pricing are subject to change. Initiation fee is due at time of registration. Membership will auto-renew each month until cancelled by the client. Members will accumulate 7 units of Botox® or Xeomin units or 21 units of Dysport each month on the date the monthly fee is paid, up to 90units per year. Members are eligible for an additional 5% discount on current published rates for all services at time of redemption. Memberships are cancelable at any time. Units prepaid can be redeemed at any time and do not expire. There is no cash value for unused units. If a membership is cancelled, applicable % discount will only be in effect during the active membership period.
